Wie muss ein ethischer Beratungsdienst strukturiert sein, damit er in moralischen Konfliktsituationen im klinischen Alltag angefragt wird und die Ratsuchenden ergebnisorientiert unterstützen kann? Nach welchen Kriterien lassen sich die Beratungsgespräche auswerten und bewerten? Zur Beantwortung dieser Fragen werden theoretisch fundierte Konzepte ethischer Fallbesprechung aus Nimwegen, Leuven und Basel herangezogen; dies geschieht vor dem Hintergrund der Erfahrungen mit der über zehnjährigen Entwicklung der ethischen Arbeit im Ev. Krankenhaus Bielefeld (EvKB). Als Resultat stellen wir einen strukturierten, multidisziplinären Ansatz vor, mit dem in (...) den Beratungsgesprächen die ethischen Dimensionen unterschiedlicher Vorstellungen herausgearbeitet und möglichst einvernehmlichen Lösungen zugeführt werden. Am Beispiel des EvKB lässt sich zeigen, dass der ethische Beratungsdienst auch in Zeiten knapper Ressourcen implementiert und praktiziert werden kann und eine Gesprächskultur schafft, die zu neuen Zielvariablen führt; hier spielen der Perspektivenwechsel und die Prozessgerechtigkeit eine zentrale Rolle und liefern zugleich Ansatzpunkte für unsere eigene Evaluationsstrategie. Zum aktuellen Stand der Evaluation klinischer Ethikberatung wird die internationale Literatur kritisch gewürdigt. Erste konzeptionelle Ansätze und Ergebnisse einer eigenen Evaluationsstrategie werden vorgestellt. Durch die Offenlegung hoffen wir, die Auseinandersetzung mit diesen Themen zu intensivieren. In der Neonatologie—wie auch in anderen Bereichen der Intensivmedizin—müssen Ärzte und Pflegende häufig die ethisch schwierige Entscheidung treffen, ob die Therapie eines schwer kranken Patienten intensiviert oder eingeschränkt werden soll. In der Literatur existieren verschiedene Instrumente zur Entscheidungsfindung, die, angewandt im Rahmen ethischer Konsile, die Nachvollziehbarkeit einer ausgesprochenen Empfehlung gewährleisten sollen. Zwei dieser verfügbaren Modelle („Schema zur ethischen Urteilsbildung nach dem Muster der aristotelischen Ethik“, Honnefelder 1994 und die „Nimwegener Methode für ethische Fallbesprechung“, Gordijn 1998) wurden anhand klinischer Fälle auf (...) ihre Anwendbarkeit in der Neonatologie hin untersucht. Unsere Studie zeigt, dass die Verwendung eines Entscheidungshilfeinstruments zwar grundsätzlich sinnvoll ist, deckt aber gleichzeitig den Konkretisierungsbedarf der oben genannten Modelle beim Einsatz in der Neonatologie auf. Der daraufhin entwickelte „Kölner Arbeitsbogen zur ethischen Entscheidungsfindung in der Neonatologie“ stellt ein auf die speziellen Bedürfnisse der Neonatologie ausgerichtetes Schema dar und könnte damit eine Lücke für den rasant expandierenden Bereich der Früh- und Neugeborenenmedizin schließen. Unter der Perspektive von Big Data werden ethische Grundfragen fokussiert, die sich aus der Entwicklung und Anwendung fortgeschrittener Informationstechnologien ergeben. Allen fortgeschrittenen informatischen Werkzeugen ist gemeinsam, dass sie mit großen Datenmengen umgehen müssen, diese zu analysieren, zu verarbeiten und möglichst selbständig auf die Analysate regulierend und steuernd zu reagieren haben. Im Fokus steht erstens die Anschließbarkeit eines speziellen Diskurses über fortgeschrittene Informationstechnologien innerhalb der Technikethik, zweitens die Verortbarkeit spezifischer Handlungsprobleme in Prozessen, die die gesamte Gesellschaft und den ethischen Status des (...) Individuums betreffen, und drittens die Aufweisung metaphysischer Voraussetzungen gegenwärtiger Einstellungen im Umgang mit Analysaten, die aus großen, insbesondere fließenden Datenmengen gewonnen werden. Es wird versucht die Kontinuität bestimmter ethischer Grundkonflikte im informationstechnologischen Diskurs aufzuweisen. Schlüsselthemen sind u.a. die Dialektik von Ermächtigung und Entmündigung im Umgang mit modernen Informationstechnologien sowie, damit verbunden, der Verlust von Widerständigkeitserfahrungen, der mit einem Wirklichkeitsverlust einhergeht. Die Studie versteht sich als Orientierungshilfe und als Beitrag zur philosophischen Verortung und Einschätzung einer Bereichsethik. Der Autor Prof. Dr. Klaus Wiegerling arbeitete bis zur Pensionierung am Institut für Technikfolgenabschätzung und Systemanalyse (ITAS) des KIT Karlsruhe. The achievement of unity and perfection in human action begins with a struggle for these ideals in human thought. Dr. Klaus Vondung in his collection of essays that span four decades explores examples of this in different fields of human inquiry: striving for harmonious existential unity of talents and morals, intellect and emotion; seeking to make natural sciences consonant with the humanities and thereby moving toward a more universal, "perfect" science; and establishing unity in political structures and cultivating in (...) this unity a homogenous society. Vondung has given special devotion to National Socialism as a context wherein he revisits its perverted motivation and the consequences of this despite noble ideals. Particular focus in following the thread of unity and perfection in human intellectual and practical ambitions ultimately hones in on the combination of religion and politics. Vondung in these essays unpacks the ways in which this continues to fascinate and disturb us, and in his expertise he uses National Socialism to connect this pursuit of unity and perfection to what he calls one of the signature marks of modernity--namely, secular apocalypticism. This claim stands in opposition to Eric Voegelin's remark that Gnosticism, rather, is "the nature of modernity." Vondung, who studied and wrote his dissertation under Voegelin, grapples with the contrast of these positions. Vondung is willing to challenge Voegelin, but ultimately his treatment of the latter bears the quality of tribute to this great scholar. Vondung also explores the points of contact between apocalypticism and Hermetic speculation. Despite the independence of the religious and philosophical doctrines of Hermeticism, there are parallels to be found. Apocalypticism and Hermeticism originated in antiquity and yet each represents a tradition that still holds footing today. Vondung furthermore leads the reader to see the project of salvation found in both even as each operates with a different scope. This collection of essays centers itself on a perspective of the human pursuit of unity and perfection, directly or indirectly, as objectives of intellectual endeavors, existential ideals, as social or political outcomes, and in the case of National Socialism even as perverse aberrations. Vondung's particular treatment of Voegelin's work likewise establishes what the former identifies as a stand-out question of this study: Does the search of order in history show us the unity of the history of humankind? BackgroundThe global COVID-19 pandemic confronts people with their fragility, vulnerability, and mortality. To date, scales to measure death awareness mainly focus on the anxiety-provoking aspect of mortality cues. This study aims to cross-culturally adapt and validate the Death Reflection Scale, a scale for measuring positive, growth-oriented cognitions of life reflection and prosocial behavior following confrontation with the finiteness of life.Materials and MethodsThe Death Reflection Scale was translated and adapted in a multi-step process to the German language. In this anonymous, cross-sectional, (...) online survey at a large university in Germany, students, healthcare professionals and other staff completed the DRS alongside comparison measures. Multi-group confirmatory factor analysis was used to assess configural, metric, and scalar measurement equivalence across four age and occupational groups. Convergent/divergent validity testing was done via Spearman correlations.Results1,703 participants provided data for a response rate of ∼5%. 24% of respondents were HCP, 22% students. Confirmatory factor analysis showed a higher-order structure of the DRS with a strong general factor and the originally proposed five subscales. Multi-group CFA showed partial metric equivalence across age groups and partial scalar invariance across occupational groups. Non-invariant scales were the Motivation to live, Putting life into perspective, and Legacy subscales. In the convergent validity testing, two hypotheses were fully confirmed, two partially and four were not confirmed. Experiencing a propensity for increased contemplation and life reflection during the pandemic together with spirituality showed correlations of moderate to large size to the DRS and its subscales.ConclusionFurther conceptual work for death awareness to explore the construct’s stability in different population groups needs to be undertaken. However, the DRS can be mostly used to assess positive and growth-oriented aspects of death awareness and death reflection which may be an important avenue when developing counseling and support interventions for groups experiencing a high burden during the pandemic. Sigieri di Brabante, maestro di filosofia presso la facoltà delle arti dell'università di Parigi, fu uno dei principali esponenti di quell'"aristotelismo radicale" che costituì uno dei principali obiettivi delle condanne del vescovo Étienne Tempier del 1270 e del 1277. Il De aeternitate mundi è la testimonianza di un corso tenuto da Sigieri nel 1272 sul problema se la specie umana sia stata creata nel tempo. Vengono qui esaminate sia in generale le tesi di Sigieri riguardo ai diversi argomenti affrontati, sia (...) nello specifico il problema della causazione per intermediari. Quest'ultimo tema rimanda non solo alle altre opere in cui Sigieri ha affrontato tali problematiche, ma anche ad una tradizione che trae le sue origini nella filosofia greca classica e si sviluppa nel medioevo latino e arabo. The aim of this study is to provide a discussion on the definitions and conceptual dimensions of Responsible Research and Innovation based on findings from the literature. In the study, the outcomes of a literature review of 235 RRI-related articles were presented. The articles were selected from the EBSCO and Google Scholar databases regarding the definitions and dimensions of RRI. The results of the study indicated that while administrative definitions were widely quoted in the reviewed literature, they were not substantially (...) further elaborated. Academic definitions were mostly derived from the institutional definitions; however, more empirical studies should be conducted in order to give a broader empirical basis to the development of the concept. In the current study, four distinct conceptual dimensions of RRI that appeared in the reviewed literature were brought out: inclusion, anticipation, responsiveness and reflexivity. Social health disparities are increasing in most countries around the world. During the past two decades, a large amount of evidence has emerged about the health consequences of social inequalities. Despite such evidence, the concept of medical fairness, as traditionally defined by the World Medical Association, has remained unchallenged and even reinforced by some scholars who emphasize that doctors should remain neutral to the socioeconomic status of their patients when providing clinical care. The inconsistency between public health and clinical care (...) perspectives raises tension between a narrow interpretation of the ethics of justice, which stresses the importance of equality and impartiality, and the ethics of care, which highlights the importance of particularity and individuality in clinical practice. This article analyzes the concept of medical fairness using the emerging evidence that demonstrates the impact of social health disparities on clinical care. It proposes a new approach of medical fairness based on equity rather than on equality and provides a consistent ethical framework based on Paul Ricoeur’s three levels of medical judgments. This integrative framework provides a better balance between public health and clinical care in medical practice. (shrink)

Societies change; and sociology has, since its inception, described and evaluated these changes. This article proposes a revised theory of collective learning processes, a conceptual framework which addresses ways in which people make sense of and cope with change. Drawing on Habermas’ classic proposal, but shifting the focus from argumentation towards storytelling, it explains how certain articulations allow for collective learning processes (imagining more inclusive orders), while others block learning processes (imagining more exclusive orders). More specifically, the article points to (...) narrative genres (romance, tragedy, comedy and irony) which organize feelings and shape the social bond, proposing that ironic and tragic stories have the potential to trigger collective learning processes, while romantic and comic stories tend to block them. We highlight the need to focus on the underlying determinants and production mechanisms to fully understand the nature of facial expression of emotion and to settle the theoretical debate about the meaning of motor expression. Although emotion theorists have generally remained rather vague about the details of the process, this has been a central concern of componential appraisal theories. We describe the fundamental assumptions and predictions of this approach regarding the patterning of facial expressions for different emotions.
